Unified Incident Management

Every event gets a record, a category, an owner, and a defined path to closure, whether it results in a review, a report, or a governance action.

It becomes the governance record for any event the organization must answer for: clinical, operational, privacy, or billing. Operational systems document the event; Abastyan documents the organizational response.

Capabilities

What the module holds and how it holds it.

Log incidents directly or upload the report from your EHR; the clinical narrative stays in the EHR. Categories follow each site’s care setting, and each is routed to its reporting clock and follow-up workflow: QAPI, breach response, or payment review.

/01
Every incident type
Clinical, operational, privacy, and billing integrity incidents in one register, with categories configured to each care setting: abuse allegations, falls, medication errors, infection outbreaks, elopements, grievances, privacy incidents, and more.
/02
Reporting clocks
Each category carries its own clock and route, from two hours for alleged abuse with serious bodily injury to state licensure and breach notification timelines.
/03
Severity scoring and overrides
Scored on harm, regulatory exposure, recurrence, breadth across sites, and remediation confidence. Immediate jeopardy and abuse allegations enter at the top of the queue regardless of score.
/04
Consequence mapping
Each incident is flagged for survey, privacy, or payment exposure and routed to the matching workflow: investigation, corrective action, breach response, or QAPI.
/05
Privacy and breach workflow
Intake, investigation, four-factor breach determination, notification, and corrective action, with individual, HHS, media, and state timelines tracked from discovery.
/06
QAPI record
For incidents that meet your QAPI criteria: root cause, corrective action, plan of correction, monitoring, effectiveness re-check, and committee review, linked to the incident.
/07
Patterns across sites
Repeat events at one site and the same category across several sites are flagged as systemic, with prior cases linked.
/08
Report ingestion, not integration
Upload the EHR incident report. Abastyan reads the organizational fields; the clinical narrative stays in the EHR. No interface agreement or vendor permission required.
/09
Hotline intake
Reports from the ARIA Hotline arrive as incident metrics, so anonymous reports enter the same register and workflows.
Defensibility

When the review comes, this is what the record produces.

What the reviewer asksWhat Abastyan produces
“Show me every abuse allegation this year and when each was reported.”
The register filtered by category, with the discovery time, the reporting clock, and the time each report was made.
“Was this privacy incident a reportable breach?”
The four-factor determination with its rationale, the decision date, and the notification log.
“How did you decide this incident did not require a performance improvement project?”
The recorded determination, with severity, recurrence, and regulatory exposure, and the closure documentation.
“Is this a repeat event?”
Repeat and cross-site patterns by category, with prior cases linked.
